2. 증상

외상과 관련된 장소, 사람, 생각, 감정 등을 의도적으로 피하려 한다.[377] 이러한 회피는 외상과 연관된 자극에 대한 지속적인 회피와 일반적인 반응 둔화로 나타나며, 다음 세 가지 이상의 증상으로 나타난다.[377]

2. 4. 인지 및 감정 변화

3. 진단 기준

국제적으로 통용되는 진단 기준에 따라 외상 후 스트레스 장애(PTSD)를 진단한다. 대표적으로 DSM-5와 ICD-11 진단 기준이 사용된다.

이러한 증상이 1개월 이상 지속되고, 사회적, 정신적 기능 장애를 일으키는 상태를 외상 후 스트레스 장애로 진단한다. 증상이 3개월 미만이면 급성, 3개월 이상이면 만성으로 구분한다.

4. 원인

이러한 외상적 사건은 반복되는 고통스러운 회상, 꿈, 플래시백 등으로 재경험될 수 있으며, 외상과 관련된 자극을 회피하고 일반적인 반응이 둔화되는 증상이 나타날 수 있다. 또한, 수면 곤란, 흥분, 분노 표출, 집중 장애, 과도 각성, 과도한 놀람 반응 등 증가된 각성 증상이 나타날 수 있다.[377]

이러한 장애는 사회적, 직업적, 혹은 다른 중요한 기능 영역에서 임상적으로 중요한 고통이나 장애를 일으키며, 호르몬 변화로 인한 두통, 식욕부진, 소화불량, 피부병 등 다양한 생리적, 임상적 현상이 발생할 수 있다.

4. 1. 신경생물학적 요인

외상 후 스트레스 장애(PTSD) 환자는 정신적 충격으로 인해 뇌 기능이 일시적으로 마비되어, 사건 전후의 기억을 회피하거나 잊는 경향이 있다. 또한, 행복감을 상실하고, 감정이 둔마해지며, 사물에 대한 흥미나 관심을 잃고, 미래에 대한 희망을 잃을 수 있다. 신체적인 증상이나 운동 능력에 문제가 생기기도 한다. 특히 학대받은 아동에게는 감정 마비와 같은 증상이 자주 나타난다.[334]

  • 항상 위험을 경계하고 쉽게 놀란다.
  • 신경이 곤두서 흥분 상태(잠을 잘 못 자거나, 짜증, 공격적, 무모하거나 자기 파괴적인 행동, 집중력 저하)를 보인다.


이러한 증상이 1개월 이상 지속되고, 가정, 직장, 사회생활에서 정상적인 기능을 하는 데 심각한 영향을 미치는 경우 PTSD로 진단할 수 있다.[333]

4. 2. 환경적 요인

7개월 전, D 씨는 친구와 함께 밤낚시를 하러 가던 중 큰 교통사고를 당했다. D 씨는 갈비뼈가 부러지는 등의 중상을 입었으나 운전하던 친구는 불행하게도 죽고 말았다. 그 이후로 D 씨는 두려움 때문에 자동차 운전을 할 수가 없을 뿐만 아니라 평소에도 신경이 예민해져서 자동차가 지나가는 소리만 들어도 깜짝깜짝 놀라게 되었다. 요즘은 죽은 친구가 나타나는 꿈을 자주 꾸면서 제대로 잠을 이루지 못하고 있으며, 교통사고 당시 피투성이 된 친구의 모습이 떠오르면서 심한 정신적 고통에서 헤어나지 못하고 있다.[377]

외상(心的外傷)의 몇 가지 예는 다음과 같다.

5. 치료

대인관계 치료는 트라우마 사건 자체가 아닌, 트라우마가 현재 대인관계에 미치는 영향에 초점을 맞춘다. 편안한 대인관계를 맺고 다양한 사회적 지지를 받도록 도와 트라우마로부터 벗어나도록 지원한다.

5. 1. 1. 인지 행동 치료 (CBT)

인지 행동 치료(CBT)는 외상과 관련된 부정적인 생각과 행동 패턴을 변화시켜 증상을 완화하는 치료법이다.

지속적 노출 요법은 치료사와의 대화를 통해 정신적 외상에 익숙해지는 심리 요법으로, 국제적으로 권장되는 인지 행동 치료의 일종이다.[337] 하지만 유효성에 한계가 있고, 기법에 정통하지 않으면 스트레스 증상을 악화시킬 수 있으므로 주의가 필요하다.

지속적 노출 요법의 구성 요소 중 하나인 현실 노출은 트라우마 기억으로 인해 트라우마와 관련된 사물, 장소, 상황에 대한 공포와 회피가 있을 때 사용된다. 치료자의 지원 하에 이러한 사물, 장소, 상황에 단계적으로 직면함으로써 "다시는 같은 피해를 입지 않는다", "지금까지 회피해 온 사물, 장소, 상황 등이 안전했다"는 깨달음을 얻어 공포감을 완화시킨다.[337]

인지 처리 요법(CPT)은 인지 재구성을 중심으로 한 치료 프로그램이다. 환자를 곤경에 빠뜨리고 회복을 방해하는 스택 포인트(자책감, 공포감, 절망감 등)를 찾아 완화하거나 수정하는 것이 목표 중 하나이다.[339] 이를 통해 "나는 잘못하지 않았다(자책감 경감)", "현재 상황은 안전하고 안심할 수 있다(공포감 경감)", "나는 형편없는 인간이 아니고 앞으로 다양한 좋은 경험을 할 수 있다(절망감 경감)"와 같은 생각을 형성하도록 지원한다.[340]

외상 집중 인지 행동 치료(TF-CBT)는 아동의 외상 치료에 사용되는 프로그램이다. 심리 교육 및 부모 역할 기술, 이완 기법, 감정 표현 및 조절, 인지 대처, 외상 이야기 및 처리, 실생활에서의 단계적 노출, 부모-자녀 공동 세션, 미래의 안전 및 발달 강화 등 8가지("PRACTICE"의 머리글자) 구성 요소로 이루어진다.[341] 유효성이 충분히 입증된 프로그램이며, 향후 보급 및 발전이 기대된다.[341]

PE는 안전한 환경에서 외상 기억에 반복적으로 노출시켜 불안 반응을 줄이는 것을 목표로 한다. 치료 도입 시에는 환자에게 트라우마 증상과 치료 원리를 자세히 설명하는 심리 교육을 실시하고, 환자와 라포(신뢰 관계)를 형성하는 것이 중요하다.[338] 불안감을 다스리기 위한 호흡법을 알려주는 것도 좋다.[338] 치료 과정 전반에 걸쳐 환자가 괴로운 경험을 공유하고 있다는 점을 염두에 두고, 지지와 공감을 표현하는 것이 중요하다.[337][338]

5. 1. 4. 기타 심리 치료

변증법적 행동 치료(DBT)는 원래 경계선 성격장애 치료법으로 개발되었으나, 외상 후 스트레스 장애(PTSD) 치료에도 효과가 있는 것으로 나타났다. 무작위 대조 시험을 통해 그 효과가 입증되었다.[344]

대인관계 치료는 트라우마 사건 자체가 아니라, 그 트라우마가 현재 대인관계에 미치는 영향에 초점을 맞춘다. 편안한 대인관계를 맺고 다양한 사회적 지지를 받도록 도와 트라우마로부터 벗어나도록 지원한다.[347] 예비 연구에 따르면, 14주간의 시험 결과 대인관계 치료를 받은 환자의 63%가 증상 점수(CAPS)에서 30% 이상 개선된 반면, 지속적 노출 치료를 받은 환자는 47%만이 개선되어 노출 없이 치료할 가능성을 보여주었다.[348]

5. 2. 약물 치료

SSRI 등의 항우울제가 외상 후 스트레스 장애(PTSD) 증상 완화에 도움이 될 수 있다. 그러나 벤조디아제핀 계열 약물은 의존성 위험이 있고 스트레스 증상을 증대시키며, 심리 치료 효과를 저해할 수 있으므로 사용에 신중해야 한다.[349][350]

2013년 세계보건기구(WHO) 가이드라인에서는 트라우마에 초점을 맞춘 인지 행동 치료나 EMDR이 실패했을 때, 또는 그러한 자원을 이용할 수 없을 때 SSRI 투여를 고려할 수 있지만, 첫 번째 선택은 아니라고 명시하고 있다. 또한, 아동 및 청소년의 PTSD에는 항우울제를 사용해서는 안 된다고 권고한다. NICE 가이드라인에서도 심리 치료를 우선 권장하며, 약물 치료를 일상적으로 사용해서는 안 된다고 명시한다.

MDMA를 심리 요법과 함께 사용했을 때 치료 결과가 향상된다는 임상 시험 결과가 있다. 미국 식품의약국(FDA)은 MDMA의 3상 임상시험을 승인했으며,[356] 영국, 캐나다, 이스라엘 등에서도 임상 시험이 진행 중이다.[357][358] 2023년 7월, 호주에서는 PTSD 치료에 MDMA 처방을 세계 최초로 승인했다.[360]

5. 3. 기타 치료

MDMA는 1985년 미국에서 범죄화되기 전까지 다양한 환각 치료에 사용되었다. 범죄화에 대응하여 심리 요법의 보조제로 사용하기 위해 MDMA를 법적 처방 의약품으로 개발하기 위한 비영리 의약품 개발 기관인 'Psychedelic Studies를 위한 종합 협회'가 설립되었다.[379] 이 약물은 두려움을 줄여 심리 요법을 촉진하고, 사람들이 감정적으로 압도되지 않고 외상적 기억을 재처리하고 받아들일 수 있도록 돕는다. 치료 과정에서 환자들은 약물의 급성 활동 기간 동안 확장 심리 치료 세션에 참여하고 치료 시설에서 밤을 보낸다. 약물 치료 세션에서 치료사는 지침을 주지 않고 환자의 내부 경험 탐구를 돕는다. 환자들은 약물 지원 세션 이전에 표준 심리 치료 세션에 참여하고, 약물 심리 요법 후에는 약물과 자신의 경험을 통합하도록 돕는다.[380] 2016년 11월 말, MDMA 보조 정신 치료제의 2상 임상 시험 결과가 발표되었다.[381]

대마초는 외상 후 스트레스 장애(PTSD)로 인한 불안과 플래시백의 영향을 약화시키고, PTSD 증상을 감소시킨다는 증거가 있다.[352] 대마초를 사용한 80명을 대상으로 한 연구에서는 증상 척도(CAPS)가 75% 이상 감소했다.[353] 합성 칸나비노이드인 나빌론을 사용한 소규모 시험에서는 악몽 치료에 사용되었으며, 47명 중 34명(72%)이 악몽의 빈도와 강도를 감소시켰고, 28명(59%)에서 악몽이 완전히 멈췄다.[354] 대마초의 유효성을 평가하기 위한 미국의 임상 시험은 2017년에 제2상 시험이 진행 중이었다.[355]

6. 1. 심리적 응급처치

외상 후 스트레스 장애(PTSD)를 예방하기 위해 심리적 디브리핑(응급 상황 스트레스 관리)이 한때 제안되었다. 이는 재해 등의 발생 2~3일 후부터 1주일까지 진행되는 집단 치료로, 2~3시간에 걸쳐 사건을 재구성하거나, 감정을 발산하고, 외상 반응에 대한 심리 교육 등이 이루어지는 방법이다.[377]

그러나 1990년대 후반부터 디브리핑의 효과에 의문을 제기하는 보고가 잇따랐으며, 현재는 고통 완화나 PTSD 예방에 도움이 되지 않으므로 강제적인 디브리핑은 중단해야 한다는 것이 일본 트라우마틱 스트레스 학회의 입장이다.[345] 2003년 일본 후생 과학 연구에 따른 '재해 시 지역 정신 보건 의료 활동 가이드라인'에서도 재해 직후의 경험을 묻는 상담은 오래된 생각에 근거하여 해로우며, 국제 학회와 미국 국립 PTSD 센터의 가이드라인에서도 비권장되고 있으므로 "해서는 안 된다"라고 기록되어 있다.[346]

6. 2. 위험군 선별 및 조기 개입

피에르 자네는 환자와의 "대화"를 통해 외상적인 사건과 관련된 참기 어려운 감정 반응이 일종의 변성 의식을 일으키고, 이 변성 의식이 히스테리 증상을 낳는다는 결론을 내렸다. 미국에서는 설리번이 특히 과각성 및 회피 증상이 나타나는 것을 밝혀내어 진단학상의 기초를 만들었다.[361]

PTSD는 개인적인 문제로 치부되기도 하지만, 사회적 지지와 이해가 필요한 문제이다. D 씨의 사례[377]처럼, 갑작스러운 사고나 재난은 누구에게나 일어날 수 있으며, 이로 인한 정신적 고통은 사회 전체가 함께 극복해야 할 과제이다.

8. 1. 한국 사회의 특수성

한국은 과거 권위주의 정권 시절의 국가 폭력, 민주화 운동 과정에서의 희생, 세월호 참사와 같은 대형 재난 등 집단적 트라우마를 경험한 역사가 있다. 이러한 사회적 맥락을 고려하여 PTSD 문제에 접근해야 한다.[377]

8. 2. 사회적 낙인 해소

현대에는 인지 행동 치료의 일종인 지속 노출 요법이 PTSD 치료에 효과가 있는 것으로 알려져 있다. 이는 안전한 환경에서 숙련된 상담자에 의해 이루어져야 하며, 그렇지 않으면 스트레스 반응을 강화할 수 있다.

제1차 세계 대전 당시 참호전 경험을 바탕으로, 전쟁 후 미국과 영국에서는 포탄 신경증(셸 쇼크) 또는 전투 스트레스 반응에 대한 연구가 시작되었다. 베트남 전쟁 이후에는 전쟁 자체에 대한 회의감으로 인한 스트레스 증상이 나타났다. 전투 스트레스 반응은 전쟁에서 정신적으로 붕괴되는 병사가 많아지면서 주목받기 시작했다. 미국에서는 전쟁에서 귀환한 병사들의 PTSD 발병률이 높아, 군 차원의 치료 가이드라인이 작성되기도 했다.

전쟁에서 병사들은 친구들의 죽음, 포로로 잡히는 것에 대한 두려움 등 극심한 공포를 경험하며, 이로 인해 히스테리 환자와 유사한 행동을 보이기도 했다. 신체적으로는 마비, 떨림 등의 증상이 나타났고, 정신적으로는 비명을 지르거나 흐느끼는 등의 증상, 또는 감정 마비, 건망증 등의 증상이 나타났다.

과거 군에서는 이러한 현상을 겁이 많기 때문이라고 보고 처벌과 협박을 동반한 전기 충격 치료를 시행했다. 그러나 진보적인 사람들은 이를 사기가 높은 병사에게도 나타날 수 있는 정신 장애로 보고 인도적인 치료를 주장했다. 이후 연구를 통해 이러한 상태에 대해 ASD나 PTSD라는 진단명이 붙여졌다.

최근에는 무인 항공기 조종사들의 PTSD 발병률이 높다는 사실이 알려졌다. 무인기 조종사는 기체에 직접 탑승하지 않아 위험에 노출되지 않고, 원격 조작으로 임무를 수행한 후 집으로 돌아갈 수 있다. 이러한 운용 방식은 조종사가 인간을 살상했다는 실감을 갖기 어렵게 하지만[362][363], 평화로운 일상과 전장을 오가는 생활, 적을 살상하는 순간을 선명한 화면으로 보는 것이 큰 정신적 스트레스를 유발한다는 의견도 있다.[364] 국제 정치학자인 P. W. 싱어는 무인기 조종사가 실제로 이라크에 배치된 병사보다 높은 비율로 PTSD를 앓고 있다고 보고했다.[365]

2012년부터 2017년까지 남수단에 파견된 일본 육상자위대원(자위대 남수단 파견)을 대상으로 한 연구에 따르면, 귀국 후 PTSD 증상을 보인 대원은 3.95%였다. 이들의 건강 조사 데이터를 분석한 결과, 수면 장애가 PTSD 발병의 공통적인 위험 요인으로 나타났다.[366]
